Sponsor forms

Congratulations on being asked to be a Confirmation Sponsor or a Godparent for Baptism. This reflects the high regard you have in the eyes of the people anticipating the celebration of these Sacraments.

Since the role of Godparent or Sponsor is one of ongoing witness of your Catholic-Christian faith commitment there are certain requirements. Please answer the questions on the form below. If you have answered "no" to any of the questions on the form and would like to meet with Fr. Roy to discuss your standing in the Church and whether or not you can become qualified to be a Sponsor or Godparent please make an appointment by calling 508-993-2351 and choosing option #3.

: We are only able to provide sponsor forms to qualified members of St. Julie's Parish. If you belong to another parish and have been asked to be a sacramental sponsor for a member of St. Julie's parish, please contact parish office of your own parish to obtain a sponsor form.
Parish ID number: This number can be found on weekly donation envelopes or via the parish office. We can only issue Sponsor Forms to members of St. Julie's parish. If you belong to another parish contact that parish office to obtain a Sponsor Form.
